***Documents needed to complete an enrollment or to prove residency if your address has changed.
PROOF OF IDENTIFICATION
Child must be 5 years old on or before September 1 in the year the student is to begin kindergarten to be eligible for admittance. School board policy does not allow for any exceptions to this date.
Please provide:
Child Identification;
- Social Security Number
- Original State Certified Birth Certificate
Parent Identification;
Parent Photo ID, e.g., Drivers License, State ID, FOID Card
PROOF OF RESIDENCY
Homeowners and Renters MUST provide Categories B, C and D. Any Current Residents on an AFFIDAVIT MUST provide from Categories A, B, C and D.
**Category A (One document required) ALONG WITH UPDATED OCCUPANCY PERMIT WITH NAMES ON IT.
*Letter of residence from landlord in lieu of lease (7:60-AP2, E1)
*Letter of residence to be used when the person seeking to enroll a student is living with a District resident (7:60-AP2, E2)
*Non-Custodial Parent/Guardian enrolling student (7:60-AP2, E3)
**Category B (One document required)
*Most recent property tax bill and proof of payment, e.g., canceled check or Form 1098 (homeowners)
*Mortgage papers (homeowners)
*Signed and dated lease and proof of last month’s payment, e.g., canceled check or receipts (renters)
*Letter from manager and proof of last month’s payment, e.g.
** Category C (MUST HAVE) (If B matches address then you need 1 from C)
*Driver’s license or State ID (the Home Owner or the Leaser)
**Category D - (Two documents showing proper address are required)
*Vehicle registration
* Voter registration
*Most recent cable television and/or credit card bill
*Current public aid card
*Current homeowners/renters insurance policy and premium
*Most recent gas, electric, and/or water bill Receipt for
*Moving van rental
*Current bank statement
- If you live with someone in our district, you and the person responsible for the residence will be required to provide necessary documents.
SPECIAL EDUCATION PAPERWORK (If Applicable)
- Individualized Education Program (I.E.P)
- Psychological Evaluation Report
- Notice of Recommended Educational Program
STUDENT IMMUNIZATION RECORDS/PHYSICALS
Forms available on the District website - www.gcsd9.net. These exams do not have to be completed by registration, but will be required the first day of school. All forms need to be taken to the School.
ADDITIONAL INFO
Students will not be allowed to attend school on the first day if not registered or enrolled. The first day of student attendance is set for Thursday, August 15. In addition, students not registered by Monday, August 5 will not receive bus transportation until Monday, August 26.
